The Office of Human Resources has issued a reminder to faculty and staff about upcoming deadlines for benefit applications. The information pertains to several important areas, including health plan contributions, dependent care subsidies, and insurance options.
Faculty and staff are encouraged to review the expanded eligibility guidelines for 2025 regarding reduced payroll contributions for health coverage. These reductions are available through Schedule B, based on household income and size. The deadline to apply is December 6. Details and applications can be found on the Schedule B webpage.
In addition, the University continues to offer dependent care subsidies as part of its efforts to support families' changing needs. Faculty and staff interested in these subsidies must submit their applications by December 6. More information is available on the Dependent Care Subsidy webpage.
For those who opted for new or increased supplemental life insurance during the annual Open Enrollment period, it is necessary to complete the MetLife Statement of Health form and return it to MetLife. Similarly, individuals who selected long-term disability insurance through The Standard must fill out a Medical History Statement form and return it accordingly.
